From Google's English: Abstract: Over the past two decades, academic publishing has undergone profound transformations with digitization and internet access. Concomitantly, the Open Access movement has played a fundamental role in improving accessibility to scientific knowledge by eliminating fees and other restrictive barriers. While this democratization of information has generated substantial benefits, it has also introduced complex challenges, turning medical publishing into a lucrative industry that tests the ethical limits of business practices and, ultimately, the integrity of research. The concentration of power in a few large publishers within a biased and inelastic market, the proliferation of for-profit publishers, and the increasing financial burden shifted onto authors are critical factors contributing to the current crisis in academia. The aim of this manuscript is to examine the main milestones in the evolution of academic publishing, analyze its current landscape, and discuss the fundamental challenges facing the industry in the Open Access era, particularly from the perspective of pediatric researchers in Latin America.
